Patient Service Agreement

Functional Movement Physiotherapy Ltd

At Functional Movement Physiotherapy, we are committed to providing the highest quality of care. In return, we ask that patients treat our staff and other service users with courtesy and respect, notify us promptly if they are unable to attend appointments, and make payments in a timely manner.

1. What to Expect From Your Physiotherapy Assessment

During your initial consultation, your Physiotherapist will ask questions about your symptoms and then carry out a physical assessment of the affected area.

To assist with your assessment:

•       Lower limb conditions: Please bring shorts to change into

•       Upper limb conditions: A vest or sleeveless top will help with the examination

•       Lower back conditions: Bring both shorts and a vest top to allow full visibility of the spine and lower limbs

If you feel uncomfortable at any time, you are welcome to bring a companion, or we can arrange a chaperone. Feel free to ask questions during your session - your Physiotherapist will be happy to explain anything.

Following your assessment, the Physiotherapist will discuss their findings with you and recommend a tailored treatment plan aligned with your goals.

2. Consent to Treatment

Before any assessment or treatment takes place, your Physiotherapist will explain the proposed care, including any material risks and reasonable alternatives, and will seek your informed consent.

Consent may be verbal or written, depending on the nature of the treatment. You may withdraw your consent, or request that treatment is paused, at any time.

You are responsible for telling us about any medical conditions, medications or other factors that might affect your treatment. With your permission, we may share relevant information with your GP or other healthcare professionals involved in your care, in line with our Privacy Policy.

3. Cancellation Policy

We send appointment reminders via email and/or SMS as a courtesy. However, it remains your responsibility to keep track of your appointments.

To cancel or rearrange an appointment without charge, please give us at least 24 hours' notice. Cancellations or changes made within 24 hours of your appointment may be charged at 50% of the appointment fee, reflecting the clinical time reserved for you.

We understand emergencies and unavoidable situations occur. If you need to cancel due to illness, hospital attendance, or other genuine emergency, please contact us as soon as you can; in these circumstances we will normally waive any cancellation fee.

If you do not attend your appointment and do not contact us in advance, we may charge the full appointment fee.

4. Referrals

If further treatment or diagnostic imaging is required, we may refer you to a third-party provider. We will supply a referral letter, but it is your responsibility to arrange an appointment with them. Once referred, you will be subject to the terms and conditions of that provider. We accept no liability for their services.

5. Direct Payments

If you are self-funding your treatment, payment is due at the time of each appointment via credit/debit card or cash. Receipts can be provided via email upon request to assist with insurance claims.

6. Medical Insurance and Payments

We can invoice your insurer directly; however, you remain responsible for full payment of your fees. We currently only work with medicash or simply heath and this needs to be with a Physiotherapist with a HCPC number.

7. Late Payments

Ongoing treatment may be suspended or cancelled if fees are not paid when due. If we are required to enforce payment, you will be liable for all associated costs.

8. Chaperone Policy

If you would like a chaperone present during your consultation, please feel free to come with a friend or family member.

9. Messaging Terms & Conditions

By opting in to receive text messages, you agree that we may send you automated SMS messages about appointments, service updates and, where you have consented, marketing about our services.

Your consent to receive marketing messages is not a condition of receiving treatment.

You can stop receiving SMS marketing at any time by following the instructions in any message (for example, replying STOP) or unsubscribing via email/ email us and we will change your status to DO NOT CONTACT. You will still receive essential service messages about your booked appointments unless you ask us not to contact you by SMS at all.

If you change or transfer your number, reply STOP or email us to update your details.

10. Confidentiality and Privacy Policy

We are committed to maintaining the confidentiality of your personal and medical information. All information you provide to us will be treated in strict confidence and will only be used for the purposes of your assessment, treatment, and care.

Your information is protected in the following ways:

•       We comply with UK GDPR and the Data Protection Act 2018

•       All staff are bound by professional codes of conduct and confidentiality

•       Your information is stored securely using encrypted electronic systems

•       We only share your information with your explicit consent, except where required by law

•       Access to your records is limited to authorised personnel involved in your care

11. Changes to Terms

We reserve the right to update or modify these Terms of Service at any time. Any changes will be communicated to you and will take effect upon your continued use of our services.
