Patient Service Plan Agreement
At Functional Movement Physiotherapy, we are committed to providing the highest quality of care. In return, we ask that patients treat our staff and other service users with courtesy and respect, notify us promptly if they are unable to attend appointments, and make payments in a timely manner. Please see the information below for further details.
What to Expect From Your Physiotherapy Assessment
During your initial consultation, your Physiotherapist will ask questions about your symptoms and then carry out a physical assessment of the affected area.
To assist with your assessment:
Lower limb conditions: Please bring shorts to change into.
Upper limb conditions: A vest or sleeveless top will help with the examination.
Lower back conditions: Bring both shorts and a vest top to allow full visibility of the spine and lower limbs.
If you feel uncomfortable at any time, you are welcome to bring a companion, or we can arrange a chaperone. Feel free to ask questions during your session—your Physiotherapist will be happy to explain anything.
Following your assessment, the Physiotherapist will discuss their findings with you and recommend a tailored treatment plan aligned with your goals.
Consent to Treatment
We will always seek your consent before beginning any assessment or treatment. Consent may be verbal or, in cases involving procedures such as injections, written consent will be required.
We aim to ensure you understand your diagnosis and treatment options. Please inform us of any medical factors that could impact your care. With your permission, we can share correspondence with your GP as needed.
Cancellation Policy
We send appointment reminders via email and/or SMS as a courtesy. However, it remains your responsibility to keep track of your appointments.
To cancel or reschedule without charge, we require at least 24 hours’ notice. Cancellations or changes made within 24 hours of your appointment will incur a late cancellation fee equal to the full self-funding rate for that session.
Referrals
If further treatment or diagnostic imaging is required, we may refer you to a third-party provider. We will supply a referral letter, but it is your responsibility to arrange an appointment with them. Once referred, you will be subject to the terms and conditions of that provider. We accept no liability for their services.
Direct Payments
If you are self-funding your treatment, payment is due at the time of each appointment via credit/debit card or cash. Receipts can be provided via email upon request to assist with insurance claims.
Medical Insurance and Payments
We can invoice your insurer directly; however, you remain responsible for full payment of your fees.
To enable billing, you must provide:
Insurance provider name
Policy number
Valid pre-authorisation code
Personal details (full name, address, date of birth, contact information)
Please note: Insurance policies may not cover all fees. You may be responsible for any excess or services not covered. If your policy limit is exceeded or a claim is denied, you will be liable for any outstanding balance, and we will invoice you directly.
Invoices are typically sent via email (or by post on request) and include payment methods, including secure SMS links via Stripe.
Late Payments
Ongoing treatment may be suspended or cancelled if fees are not paid when due. If we are required to enforce payment, you will be liable for all associated costs.
Chaperone Policy
If you would like a chaperone present during your consultation, please inform reception or speak to your clinician. We are happy to arrange this for you.
Messaging Terms & Conditions
By opting into our messaging service, you agree to receive recurring automated text messages from Functional Movement Physio regarding appointments, services, and marketing. These messages may be sent via automated systems to the mobile number you provide.

Your consent to receive messages is not a condition of purchase.
Cancellations
To unsubscribe, follow the instructions in any message (typically replying STOP). You will receive a confirmation message, and no further texts will be sent unless you opt in again.
