Carpal Tunnel physiotherapy at a Liverpool clinic

Pins and needles in your hand?

Carpal tunnel syndrome happens when the median nerve is compressed at the wrist, causing tingling, numbness, and weakness in the thumb, index, and middle fingers. It is often worse at night. For mild to moderate cases, conservative physiotherapy resolves the majority without surgery.

  • Pins and needles or numbness in the thumb, index, and middle fingers
  • Worse at night waking you and easing when you shake the hand
  • Weak grip dropping things or struggling with fine tasks
  • Aching into the forearm sometimes up from the wrist
  • Worse with repetitive use keyboard, tools, or gripping
  • Told it is splints or surgery with no mention of physiotherapy

Causes and types

About carpal tunnel syndrome

Getting the specific cause right is what makes treatment work. We assess and treat:

Median nerve compression

The nerve squeezed in the carpal tunnel at the wrist.

Repetitive hand use

Keyboard, tools, and gripping increasing pressure.

Wrist posture

Sustained bent-wrist positions raising tunnel pressure.

Fluid and swelling

Including during pregnancy, which often resolves after.

Associated conditions

Diabetes and thyroid issues increasing risk.

Mild to severe

Ranging from occasional tingling to constant numbness and weakness.

Our approach

How we treat carpal tunnel

01

Accurate assessment

We confirm it is carpal tunnel and gauge severity, ruling out neck-related or other nerve causes that mimic it.

02

Nerve and hands-on work

Nerve gliding (mobilisation), manual therapy, and specific exercises to reduce pressure on the median nerve and ease symptoms.

03

Fix the cause

Ergonomics, wrist posture, splinting where helpful, and load management, so the nerve has room to settle.

The third option

Carpal tunnel without surgery

Most people with carpal tunnel are told their only options are splints or surgery. For mild to moderate cases, structured physiotherapy is an effective first step that resolves the majority.

Nerve mobilisation

Specific gliding and manual therapy to take pressure off the median nerve.

Ergonomic change

Wrist posture, workstation, and load adjustments so the nerve can settle.

Carpal Tunnel FAQs

Can carpal tunnel be treated without surgery?

For mild to moderate cases, yes, conservative physiotherapy resolves the majority. Surgery is reserved for severe cases or those that do not respond. Physio should usually be tried before surgery for milder cases.

How long does it take to improve?

Mild to moderate carpal tunnel caught early often improves over 6 to 12 weeks of focused treatment. More established or severe cases take longer and may not fully resolve without surgery.

Why is it worse at night?

At night the wrist often bends and fluid settles, increasing pressure in the carpal tunnel, so symptoms flare and wake you. Shaking the hand temporarily relieves it.

Will I definitely need surgery?

Not for most mild to moderate cases. Surgery is for severe nerve compression or cases that have not responded to conservative care. We will be honest about when it is the right step.
