The rotator cuff is four muscles that stabilise and move the shoulder. Cuff problems, from tendinopathy and irritation to partial and full tears, are one of the most common causes of shoulder pain. The good news is that most rotator cuff problems, including many tears, respond well to physiotherapy without surgery.

Rotator Cuff FAQs

Do rotator cuff tears need surgery?

Not usually. Most rotator cuff problems, including many partial and even some full-thickness tears, respond well to progressive strengthening. Surgery is reserved for large or traumatic tears or cases that do not respond.

How long does a rotator cuff injury take to recover?

It varies with severity. Tendinopathy and irritation often improve over 6 to 12 weeks of loading. Tears and post-surgical rehab take longer. We give you an honest timeline at your assessment.

Can physio help a tear I already have on a scan?

Often yes. Scans show cuff changes in many pain-free shoulders too, so the scan is not the whole story. Strengthening the cuff and shoulder blade improves function and reduces pain in many people with tears.

Why is it worse at night?

Rotator cuff problems commonly cause night pain, especially lying on the shoulder, as the position loads the irritated tendons. Improving the cuff usually settles this.
